Each ZIP is classified by Census division and by urban, suburban, rural, or frontier locality. Those factors scale the billed-charge components below — they are not a single flat multiplier on a national average.
Ground is a response base plus loaded mileage. Helicopter is a lift-off fee plus billed flight miles (loaded miles plus a typical positioning leg from the aircraft’s base). Flight-time complexity grows with distance, which is why helicopter totals jump faster than ground. Fixed-wing is an aircraft launch, cruise miles with a billed minimum, and two ground-ambulance airport transfers. It has a high floor even on short trips and becomes the more relevant air option past about 50 miles.
The high end of each range widens with distance and in rural or frontier areas because wait time, specialty teams, and deadhead positioning vary widely.
